MNOHS Withdrawal Form

If you are a comprehensive (MNOHS-only) student or a full-time tuition student at MNOHS and would like to end your enrollment, please follow these two steps:

  1. Fill out all parts of this form and submit it
  2. Send a confirmation email from the parent/guardian email address MNOHS has on record

PLEASE NOTE: If you choose the fifth answer above (leaving high school), you are agreeing to the following: ” I hereby elect to withdraw my child, from Minnesota Online High School (and all public-school settings which are subject to Minnesota compulsory education laws) effective the withdrawal date entered above pursuant to Minnesota Statutes Chapter 120A.22, subd. 8. I have met with school personnel and have been informed of other educational opportunities, including alternative educational options, and have been provided with available information on those opportunities in order to pursue them if we choose to do so.”
